Get message when i want to ad a second database

Hi,
Got a little problem on my siteworx account.
I have create true nodeworx a siteworx account, with a capacities of 20 databases now the problem am facing is when i want to ad a second database with the same user under the same account i get a message:

There was a problem validating the form. Please see details below.

but i don`t see the problem..
the only thing i want is to ad a second database..
how to resolve this?
